Forms for individual charities
We have five forms for individual charities to use. They are the:

Application for registration
Use this form to
apply for registration as an individual organisation.

Officer certification
Use this form to
certify that each of your charity's officers is not disqualified from being an officer under the Charities Act. Use this form when you apply for registration, and then whenever you appoint a new officer.

Annual Return
Use this form to
file an Annual Return.

Notification of changes
Use this form to
report changes to your charity’s name, address, balance date, rules, purposes, officers.

Notification of change for a disqualified officer
Use this form to
let the Charities Commission know that a certified officer has become disqualified from being an officer under the Charities Act.
Forms for groups
Some forms have been designed specifically for charities registered as a Group (two or more charities registered together). They are the:

Group member information
Use this form to
provide Charities Services with information about each member of the group. Use this form when making a submission to Charities Services for two or more organisations to be considered as single entity under the Charities Act.

Group application for registration
Use this form to
register a group of organisations as a single entity.

Group Annual Return
Use this form to
file an Annual Return for a group. The parent entity may also have to file a Form 4 for its group members, depending on the terms and conditions set for the group.

Group notification of changes
Use this form to
report changes to a group, or one of its members, where such changes impact on the information we hold in relation to the single entity. The terms and conditions set for each group specify how the group parent should report changes to the details of the single entity.
