Forms
The Charities Act directs the Commission to collect information from charities. We encourage you to complete our forms online, rather than on paper, by logging in to your account. Using online services, you can apply to register, certify officers, notify changes and file Annual Returns. You will also receive important updates and communications from us by e-mail.
If paper forms are your preferred option, please read the information below and then choose the paper form you need from the list in the right pane of this page.
Forms for individual charities
We have four forms for individual charities to use. They are the:
- Application for Registration — Form 1
- Officer Certification — Form 2
- Notification of Changes — Form 3
- Annual Return — Form 4
Forms for groups
Some forms have been designed specifically for charities registered as a Group (two or more charities registered together). They are the:
- Group member Registration — GMI
- Group Application for Registration — Form 5
- Group Notification of Changes — Form 6
- Group Annual Return — Form 7
